Tampa, Fla. (CSX) August 29, 2003
At approximately 2100 hrs. in CSXT's Yeoman Yard, remote control assignment y-291-29 was instructed to pull a heavy cut of cars from a track at the South end of Yeoman yard and shove to another track.

Y-291-29 apparently was reportedly having communication problems all afternoon. This pull and shoving move constituted this assignment to get the signal at TS (Tampa South), which brought the lead end of this cut on the main line (Tampa Terminal subdivision) over Highway 60 (Adamo Drive). At this point, it was reported that another com-loss occurred.

Highway 60 was blocked for over 1 hour.

Also, during the tour of duty, the locomotive horn blasted without initiation. The Remote Control Operator reported an injury to the Trainmaster on duty. (Riding front of locomotive with horn blasting without initiation.)
