A remote control operator was injured due to a malfunction of the transmitter box worn by the RCO, which controls the locomotive horn. In Tampa, the locomotive horn often blasts uncontrollably without initiation while in remote control operation. This safety hazard had been reported to CSX managers prior to the incident.
